Warning Signs After a Chiropractic Adjustment

Most people feel fine after a chiropractic adjustment. Some may notice temporary soreness, stiffness, a mild headache, tiredness or discomfort around the area that was treated. These reactions are usually mild and short-lived.

But what if you feel worse after an adjustment? When is that part of a normal response, and when should you call your chiropractor or seek medical attention?

Dr. Logan Osland sees these questions regularly. “A little soreness or stiffness after an adjustment is not necessarily a reason for concern,” says Dr. Osland. “What matters is the severity of the symptom, whether it is getting better or worse, and whether you develop symptoms that are outside the usual pattern.”

What Are Normal Symptoms After Going to the Chiropractor?

Mild, temporary symptoms can happen after spinal manipulation. The most common include soreness, stiffness, localized discomfort and a mild headache. Some people also feel tired for a short time after treatment, similar to some other wellness treatments. According to the National Center for Complementary and Integrative Health, these side effects generally resolve within about 24 hours.

You may notice these symptoms in the muscles or joints that were treated, particularly if the adjustment addressed an area that has been stiff, irritated or painful for some time.

Soreness or Stiffness

Muscles and joints can feel tender after an adjustment, much like they might after unfamiliar physical activity. The discomfort should generally be manageable and should begin to improve rather than progressively intensify.

Headache After a Chiropractic Adjustment

A mild headache can occur after spinal manipulation, particularly when the neck or upper back is treated. Temporary headaches are recognized as one of the more common minor side effects of spinal adjustments.

A mild headache that improves with time is different from a sudden, severe or unusual headache. If a headache is intense, rapidly worsening or accompanied by neurological symptoms such as weakness, difficulty speaking, vision changes or significant loss of balance, seek emergency medical attention.

Feeling Tired After an Adjustment

Some people feel fatigue after a chiropractic appointment. Tiredness has been reported as a temporary side effect of spinal manipulation and does not necessarily mean something went wrong.

If you feel tired, give yourself some time to recover and pay attention to how you feel over the following hours. Extreme or persistent symptoms deserve a conversation with your healthcare provider.

Feeling Weird After a Chiropractic Adjustment

“Feeling weird” is difficult to define because patients can mean many different things by it. You might feel unusually relaxed, tired, lightheaded, sore or simply different from how you felt when you walked into the office.

A temporary change in how you feel does not automatically mean there is a problem. At the same time, it is important not to dismiss significant symptoms simply because they happened after an adjustment.

Dr. Osland recommends looking at the overall picture. “If you feel a little different but are otherwise comfortable and the feeling is passing, you can monitor it,” he says. “If something feels significantly wrong, especially if it is getting worse, call the office or seek medical care depending on the symptoms.”

Dizziness deserves particular attention. Mild dizziness can have many causes and is not automatically a sign of a serious complication. However, new or severe dizziness, especially when combined with severe headache, neck pain, vision problems, difficulty walking or other neurological symptoms, should be evaluated promptly.

Nausea or Feeling Sick After an Adjustment

Some patients report nausea or a general feeling of being sick after chiropractic treatment. Nausea can have many possible causes, and it should not automatically be explained as the result of “toxins being released.”

Mild, short-lived nausea may simply pass. However, persistent vomiting, severe nausea, fainting, significant dizziness or neurological symptoms are reasons to seek medical advice.

Back or Neck Pain After an Adjustment

An adjustment is intended to address pain and improve movement, but temporary discomfort can occur afterward. Mild back pain, muscle soreness or stiffness may be noticeable for a short period, particularly when an area has already been irritated.

Nerve Pain After a Chiropractic Adjustment

Nerve-related symptoms deserve more attention than ordinary muscle soreness.

Nerve pain may feel sharp, burning, shooting or electric. You might also experience tingling, numbness or a feeling of weakness in an arm or leg. These symptoms can have many causes, including an irritated or compressed nerve, and the cause cannot be determined from the symptom alone.

Mild, temporary changes may not indicate a serious problem, but new or worsening numbness, tingling, radiating pain or weakness should be reported to your chiropractor or another healthcare professional.

Weakness is particularly important. If you suddenly cannot lift an arm, move one side of your body normally or control your movements, do not wait to see whether the symptom improves. Seek emergency medical attention.

How to Relieve Pain After a Chiropractic Adjustment

For mild soreness or stiffness, simple measures may make you more comfortable.

Gentle movement can help keep the treated area from becoming unnecessarily stiff. Avoid jumping straight into heavy lifting, intense exercise or other activities that place significant stress on a sore area.

Depending on the type of discomfort, a cold or warm compress may also provide relief. Follow the specific aftercare instructions given by your chiropractor, since the appropriate approach can vary depending on the treatment and your condition.

The most important point is not to push through worsening pain. If discomfort becomes severe, lasts longer than expected or continues to get worse, contact your chiropractor for advice.

What Are Red Flags in Chiropractic?

Most post-adjustment symptoms are minor, but some symptoms need prompt medical evaluation.

Call the chiropractic office if you have significant pain, symptoms that are getting worse instead of better, persistent numbness or tingling, new weakness, or symptoms that concern you even if you are unsure whether they are related to the adjustment.

Keep in mind that some symptoms require emergency care.

Signs of Stroke After a Chiropractic Adjustment

Stroke after a chiropractic adjustment is extremely rare, but it is important to know the warning signs. Neck manipulation has been associated with rare cases of cervical artery dissection, a tear in the wall of an artery in the neck that can lead to stroke. The evidence does not establish that cervical manipulation causes these events in every reported case, and the overall incidence appears to be low.

The reason to know the warning signs is simple: stroke treatment is time-sensitive.

According to the CDC, sudden symptoms can include:

  • Weakness or numbness of the face, arm or leg, particularly on one side
  • Sudden trouble speaking or understanding speech
  • Sudden vision problems
  • Sudden difficulty walking, dizziness or loss of balance
  • A sudden severe headache with no known cause

A severe headache or neck pain accompanied by neurological symptoms also deserves urgent attention. The American Heart Association notes that cervical artery dissection can present with symptoms such as neck pain, headache, dizziness and other neurological problems.

If these symptoms appear after an adjustment, especially after treatment involving the neck, call 911 immediately. Do not wait for the symptoms to disappear and do not drive yourself to the hospital.

Dr. Osland emphasizes that this warning is about recognizing symptoms, not creating unnecessary fear. “Serious complications are very uncommon,” he says. “But if someone develops signs of a stroke, the right response is to treat it as an emergency.”

When Should You Call the Chiropractor?

Contact the office when you are experiencing a symptom that is concerning but does not appear to be an immediate emergency.

That can include:

  • Pain that is severe or continues to worsen
  • Persistent or increasing numbness or tingling
  • New nerve pain
  • New weakness that is not severe or sudden
  • A headache that is unusual or does not improve
  • Dizziness that persists or concerns you
  • Symptoms that are not improving within the expected timeframe
  • Any reaction that makes you uncomfortable about continuing treatment

Your chiropractor can ask about what you are experiencing and determine whether you should come back for an evaluation or seek care elsewhere. You do not have to diagnose the problem yourself before calling.

When Should You Seek Urgent Care?

Some symptoms should not wait for a chiropractic appointment.

Seek emergency medical attention for sudden weakness or numbness, facial drooping, difficulty speaking, sudden vision changes, severe loss of balance, a sudden severe headache or other signs that could indicate a stroke. The CDC recommends calling 911 immediately when stroke symptoms occur.

Severe difficulty breathing or swallowing, loss of consciousness, or rapidly worsening neurological symptoms also warrant immediate medical evaluation.

These symptoms are not something to monitor at home simply because they occurred after a chiropractic adjustment.

For most people, the best approach is simple: follow your chiropractor’s aftercare instructions, stay reasonably active without overdoing it, give your body time to recover and pay attention to how you feel.

Mild soreness, stiffness, headache or tiredness can occur after spinal manipulation and usually resolve quickly.

You do not need to assume every unusual sensation is dangerous. At the same time, you should not ignore severe or worsening symptoms.

Stroke and other serious complications following spinal manipulation are very rare, but knowing the warning signs matters. Sudden weakness, facial drooping, speech problems, vision changes, severe balance problems or a sudden severe headache require emergency medical attention.

Minor side effects can appear during the first several hours after an adjustment and may sometimes be noticed later that day.
